Policy Overview

• Delivery: Records will be delivered via download from secure web site after being processed. This should typically be done within 2 weeks, but please allow us a month.
• Refunds: There are no refunds once record requests or medical services have been rendered. The only exception is for over payment for services after both inssurance and patient resonsiblity have been reconciled. These refund requests will be processed within 60 days.
• Cancellations & Returns: Record requests (and other services) cannot be cancelled or returned for a refund after being created.
• All record requests must have a valid Record Release or Subpoena for processing. Subpoena's are subject to HIPAA required validation.
